The undersigned is the parent and/or legal guardian of name above, a minor, who is now under my control and custody. I grant my child permission to participate in any activities sponsored by or conducted by Junior Assembly of North Dallas, a volunteer, non-profit, 501 (c) (7) organization. In consideration of my child being permitted to participate in such activities, I hereby release Junior Assembly of North Dallas and all those associated with same, including its officers, board of directors, employees, agents, representatives, volunteers and host locations from any and all liability and responsibility in connection with such activities and hereby release all of said parties from liability by reason of any accident or injury suffered by said child while attending such activities; provided, however, no party is released from wrongful or grossly negligent acts.
I hereby grant Junior Assembly North Dallas permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of Junior Assembly North Dallas and will not be returned. I hereby irrevocably authorize the Junior Assembly North Dallas to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge Junior Assembly North Dallas from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
